Service Overview:

The Community Liaison Service operates within multiple hospitals, with a primary emphasis on benefiting individuals under the care of the Mental Health Services for Older People (MHSOP) team within the Cardiff and the Vale Health Board.

The purpose of this service is to promote both mental and physical well-being, strengthening independence and self-assurance, while encouraging active community engagement that fosters a profound sense of belonging and connection.
